Care Manager

We are looking to hire an enthusiastic care manager to create personalized care plan for patients. The care manager will be responsible for all aspects of the day-to-day operations within the care setting.

 Care manager responsibilities:

• Creating and updating individualized care plans for patients.
• Collaborating with patients’ families to develop treatment plans
• Identifying and hiring high-quality healthcare providers
• Ensuring patients’ treatment requirements are met
• Provide information, advice, and support to residents’ families
• Ensure the delivery of quality, a person-centered care.

Job requirements:

Bachelor’s degree in social science, social work, nursing, or a related field

2+ years work experience in public health or a related field

Registered nurse (RN) license

Certified case manager (CCM) license preferred.

Outstanding communication skills, both written and verbal

Ability to travel locally
