Job requirements:
- Bachelor’s degree in social science, social work, nursing, or a related field
- 2+ years work experience in public health or a related field
- Registered nurse (RN) license
- Certified case manager (CCM) license preferred.
- Outstanding communication skills, both written and verbal
- Ability to travel locally.
